Can these be used to replace actual windows that have rotted out?

I would likely require some onsite engineering since they are not shutters, which means that light will not be able to pass through them and they are also not double sided. They are mean to be used as a decorative accent piece above the door/window.

Do they have to be painted?

All Fypon polyurethane products come with 2 coats of a white primer and must be painted or it will void the warranty (the primer itself would crack from UV exposure). Painting with an exterior grade 100% acrylic latex paint is recommended.

Do you need to order two keystones (one for each side) to properly install the sunburst pediment over a front door crossbeam? What purpose do they serve?

The keystones are optional, and typically you would only order one keystone and mount it in the center. They are purely for decoration and not necessary for the installation.
